Privacy Policy

This site collects very little, and this page explains exactly what and why. Quebec's Law 25 governs how personal information is handled here.

Who is responsible

This website belongs to the pediatric physiotherapy practice of Aliza Sarah Friedman (practising as Aliza), serving the Greater Montreal area.

Under Quebec's Act respecting the protection of personal information in the private sector, a business must name the person responsible for protecting personal information. That person is Aliza Sarah Friedman, reachable at sarfriedman@gmail.com or 514 812 1797.

What this website collects

Only what you type into the contact form and choose to send:

  • Your name
  • Your phone number and email address
  • Your child's age
  • Your preferred setting (home, school or daycare)
  • Whatever you write in the message box

Nothing is collected if you simply read the site and leave.

What this website does not do

Stated plainly, because most sites cannot say this:

  • No cookies. None at all.
  • No analytics or tracking. No Google Analytics, no advertising pixels, no session recording.
  • No profiling, no advertising, no automated decisions about you.
  • Nothing is ever sold, rented or traded.

Why it is collected, and your consent

Solely to reply to you, arrange a call or a visit, and answer your questions. Sending the form is how you give consent, and it is the only purpose the information is used for. You are never added to a mailing list.

Who else sees it, and where it goes

Form submissions are delivered by Web3Forms, a form-delivery service acting on our behalf, and then arrive in an email inbox. Both providers operate servers outside Quebec, in the United States, which means your information is communicated outside the province and is subject to the laws where it is stored.

Law 25 requires this be disclosed, so it is. Nobody else receives your information. If you would rather not use the form, phone 514 812 1797 or email directly instead.

A note about health information

The message box asks you to keep things general and save the details for the call. That is deliberate: a web form is not the right place for detailed information about a child's health.

Once you become a client, clinical records are something separate. They are kept confidentially under the professional obligations that apply to physiotherapists in Quebec, including those of the Ordre professionnel de la physiothérapie du Québec, and are not governed by this website policy.

How long it is kept

Enquiries that do not become clients are deleted within roughly twelve months. If you become a client, your contact details are kept with your file for as long as professional record-keeping obligations require, and then destroyed.

Your rights

You may at any time ask to:

  • see the personal information held about you
  • correct anything inaccurate or incomplete
  • withdraw your consent
  • have your information deleted
  • receive it in a portable, machine-readable format

Write to sarfriedman@gmail.com and you will get an answer within 30 days. If you are unhappy with the response, you may complain to the Commission d'accès à l'information du Québec.

Changes

If this policy changes, the date below changes with it. There is no mailing list to notify, so the current version is always the one on this page.
